2011-12 Editable Student Entry Form
The 2011-12 Student Entry Form and Free/Reduced Lunch Form is now available in an editable .pdf format. Please be aware that you cannot save changes to these forms. The entire form can be edited on the computer and then printed (at the time of completion) for your signature. However, if you have multiple children you can complete the forms for one child, print it out and then edit necessary fields for other children in the household.
Paper forms will continue to be available at all schools and the district office, or you may simply print out the blank .pdf form.
